Job Description:
SUMMARY OF POSITION: - Provide Case Management Services to
individuals with developmental disabilities. - It is the
responsibility of the Case Manager to facilitate and assist
individuals and teams in developing a compliment of services that
will meet the ever-changing supports of the individual or family
being served. Further assure that the ISA is implemented and
modified as determined by each team.SPECIFIC DUTIES AND
RESPONSIBILITIES:
- Develop services and coordinate the
implementation of ISA, strongly fostering self-determination for
each individual.Provide or obtain technical assistance as requested
by the team and/or Services Specialist to establish a network of
resources to support the team.Mediate and maintain objective
viewpoint when dealing with situational difficulties and assist in
reaching a resolution.Assure communication with all team members by
disseminating information and when necessary acting as a liaison
between team members.Assure that the ISA is a working document that
supports the individual in obtaining goals and services. - Further,
that it meets all technical compliance requirements as outlines by
the Division of Disability and Aging Services.Coordinate and attend
staff and/or Developmental Home Provider meetings/ visits and
trainings to monitor changing needs and expand skill of support
providers in specific areas as needed.Assure accountability by
adhering to the financial and service parameters as set forth in
Medicaid Waiver or through targeted case management.Assist in the
interview process for Support Staff and Potential Home Providers.
-Attend approved trainings, in-services, and/or conferences to
assure professional growth.
- Coordinate coverage for services during support staff time off
(e.g. vacations and sick time).
- Provide coordination and aid in emergency situations when
assigned. - Participate in on-call responsibilities as determined
by each Community Support Services division. On-call coverage may
be expectedAssure that all team members are aware of and complete
delegated responsibilities as required.Provide training and support
to individuals who use specialized communication systems such as
augmentative & alternative communication and Facilitated
Communication.Provide regular supervision of Service Specialist and
team meetings.Adhere to all Washington County Mental Health
Services policies and procedures. Assist with lifting and transfer
of clients as determined by their physical needs.Additional duties
as required or assigned by your supervisor.QUALIFICATIONS: - - - -

Bachelors Degree in a related field with a minimum of 3 years
experience working with individuals with developmental disabled,
and supervisory experience. - Possession of a driver's license,
excellent driving record and access to a safe, reliable, insured
