Seeking a Physical Therapist who willdetermine appropriate care and treatment for a geriatric population at a widerange of functional levels. You will maximize the quality of life and movementpotential within the spheres of promotion, prevention, diagnosis, treatment orintervention, and rehabilitation. You will create and implement uniquetreatment plans for the patient's current level of functioning as well aspotential improvement, while demonstrating superior clinical judgment. Compensation: $80,000 - 85,000 with up to $15,000 productivity. Responsibilities Include:Provide direct patient care, evaluate functional needs and outcomes, and consult with other specialists. Review patients' functional needs and adjust care plans to ensure optimal outcomes.Design and implement plans of care for patients based on a thorough assessment of potential development; Plans will include but are not limited to manual therapy, aquatic therapy, neuromuscular training, modalities and functional rehabilitation. Conduct therapy practice within defined standards of careCreate fitness and wellness-oriented programs tailored to patients' specific needs; Promote patient's healthy lifestyle by improving strength, flexibility, balance, and coordination.Communicate effectively with other professionals and support staff to achieve positive patient outcomes - Document patient care services via EMR Education: Graduate of a CAPTE accredited school of physical therapyActive or pending state license to practice physical therapyPassion for delivering quality, evidence-based patient careSuperior customer service and interpersonal communication skillsAbility to manage and interact positively with a diverse patient population of all ages
