POSITION SUMMARY
The Medical Records Specialist responds to all medical records
requests including subpoenas, patient/client record requests, and
third-party record requests in accordance with all applicable state
and federal privacy laws and regulations and established protocols.
Performs all duties associated with the management of electronic
and paper-based multidisciplinary medical records (e.g., behavioral
health, primary care, and dental records). Performs document
scanning, categorization and directing of scanned reports to
ordering provider/care team and data entry. Ensures all records
security and retention requirements are met including record
archiving and retrieval. Delivers professional internal and
external customer service including answering phones and emails and
advising on release of information requirements and procedures.
Responds to subpoenas, including record preparation, coordination
with attorneys, courts, and subpoenaed staff, and delivers and
authenticates records at court as needed. Able to work in a
fast-paced environment, interact with patients/clients, caregivers,
medical personnel, state agencies, and legal offices. Able to
utilize reports and technology (i.e. electronic health record or
other applicable digital operating systems), prioritize multiple
competing demands in an organized and efficient manner.
EDUCATION AND EXPERIENCE/QUALIFICATIONS
High School Diploma is required, Associates degree is preferred.
Proficiency in reading, handling, and scanning electronic medical
and/or other records or official documentation is required.
Possesses working knowledge of relevant software and has experience
with Microsoft Office applications. Ability to speak and read
Spanish is preferred. Registered Health Information Technician
(RHIT) certification or other health information certificate is
preferred. Must be capable of working flexible hours and have
access to reliable transportation.

ESSENTIAL DUTIES AND RESPONSIBILITIES
Receives, logs, and accurately prioritizes and processes all
internal and external requests for multidisciplinary medical
records in a timely manner and in accordance with established
policies, procedures and applicable state and federal privacy laws
and regulations. Determines the appropriate documents to release
which satisfy the purpose of the record request.
Receives, logs, and accurately prioritizes and processes all
subpoenas in a timely manner and in accordance with established
policies, procedures and applicable state and federal privacy laws
and regulations. Interacts with attorneys, legal offices and state
agencies. Provides guidance to subpoenaed staff regarding the
subpoena process to ensure appropriate protection of sensitive
information within the health record. Delivers and authenticates
records at court as needed.
Appropriately manages and assists with accurate and timely
escalation and response to external audits, inspections, and
reviews by Wheeler regulators, payers and governmental bodies.
Prepares, prints/scans requested documents for submission.
Provides effective professional customer service via telephone,
written, and face-to-face interactions with individuals including
patients/clients, outside individuals, and Wheeler staff seeking
information on accessing and/or managing medical records.Gives
accurate guidance on release of information procedures and related
forms.
Understands and adheres to all state and federal confidentiality
laws and regulations and ensures the confidentiality and
appropriate storage and retention of all private and protected
health information in accordance with such laws/regulations and
Wheeler polices. Ensures patients'/clients' rights to access,
release and amend their medical record met.
Performs all duties associated with multidisciplinary medical
record management, including record archiving, labeling/indexing
and record retrieval, including interacting with off-site record
storage vendor system. Able to work within multiple internal and
external and/or legacy electronic record systems and portals.
Performs management of efax document imaging/scanning, analysis,
categorization and directing of scanned reports and documents to
the appropriate ordering provider/care team. Determines need for
and performs necessary data entry.
Utilizes the electronic health record and other available
technologies such as reports and spreadsheets to carry out, track,
and streamline daily tasks.
Achieves established productivity standards as set within the
Central Records Unit.
Demonstrates the ability to work as part of a team and contributes
to the overall effective operation of the Central Records Unit and
coordinates across Wheeler departments to support the overall
mission of the organization.
Continues to develop knowledge and understanding about the history,
traditions, values, family systems, and artistic expression of
groups served as well as uses appropriate methodological
approaches, skills, and techniques that reflect an understanding of
culture.
