Description Job Schedule: Full Time Standard Hours: 40 Job
Shift: Shift 1 Shift Details: LPN will be working out of the clinic
at the Rose City Senior Center 2-3 days a week. Will report to

our communities. Job Summary A member of the Community Health
Department (CHD,) a licensed practical nurse (LPN) responsible for
provision of an organized program directed toward community
education and health improvement. Utilizes nursing skills to
analyze, develop, and implement community education programs and
community screenings based on departmental priorities. Assists with
staff education related to caring for people from different
cultures and diversity issues. Works in conjunction with other
members of the CHD.Qualifications Experience: Two (2) years
previous LPN nursing experience required. Five (5) years previous
nursing experience in community health preferred. Licensure,
Certification, Registration: Current State of CT LPN license, BLS
certification and valid drivers license with clean record required.
