Job Description:
POSITION SUMMARY: The Cardiac Rehab Staff Nurse reports to the
MMH Cardiac Rehabilitation Site Manager.# The Registered Nurse
supervises medically prescribed exercise therapy appropriate to
client#s age, medical and emotional/mental status.# The RN provides
education classes, concentrating on client#s understanding of the
cardiovascular system and heart-healthy life style modifications
necessary to maintaining optimum health.# The Cardiac Nurse is
accountable for staffing in all areas of patient services under
Cardiac Rehab/Cardiac Stress/TEE and is responsible for marketing
of program, orientation of the cardiac patient and participating in
Quality Improvement. The Cardiac Nurse will participate in the
operational duties of the department, such as but not limited to:#
Quality Improvement, Policy and Procedure maintenance, revising and
development of billing, gathering of statistical data, developing
departmental educational opportunities and participating in the
on-going growth and maintenance of the excellent customer services
provided to all that ECHN serves. # EDUCATION/CERTIFICATION The
Registered Nurse must be a graduate of an accredited professional
nursing education program Have a current Connecticut license.
Required education includes Certification in cardiovascular
clinical care (coronary care course) and current BCLS and ACLS
certification.# # EXPERIENCE Must have a minimum of two years
recent acute/emergent care experience. # COMPETENCIES The incumbent
must be proficient in cardiac monitoring and be sedation conscious

to the Cardiac Rehabilitation Program. Exemplary:# Markets directly
to physicians or physician office to facilitate client entry into
Cardiac######### Rehabilitation Program. Participates in community
education by interacting regularly with the community thereby
increasing awareness of the prevention and treatment of heart
disease.# Actively represents ECHN at rehabilitation related
professional organizations. Exemplary:# Seeks opportunity to
represent ECHN Cardiac Rehabilitation Department as a public
speaker to community organizations. 3. Assesses the patient#s
condition and care needs by collecting accurate and thorough data
on admission and on an ongoing basis through the performance of
cultural and psychosocial assessment, including review of the
medical record as per department policy. Exemplary:# Anticipates
changes in the patient#s condition, based upon patient assessment
data and recognizes subtle abnormalities in the patient#s condition
and anticipates the effect on outcomes. 4. Accurately identifies
actual and potential nursing diagnoses and/or problems. Exemplary:#
Groups and analyzes assessment data to identify specific patient
issues or needs with consideration of interdisciplinary issues in
the formulation of nursing diagnoses. 5.# Assesses patient and/or
family learning needs, including the ability and readiness to learn
and# identifies and arranges appropriate resources to assist in the

needs, including specialized discharge concerns. 6.# Accurately
completes necessary documentation of all aspects of the nursing

nursing actions to ensure individualized patient-centered care by

needed. 8.## Begins discharge planning on admission and updates it
throughout the hospitalization in addition to updating the
discharge plan frequently and keeping the Case Manager well
informed of patient needs. Exemplary:# Anticipates complex
discharge needs and intervenes appropriately. 9.# Appropriately
delegates duties and tasks to team leaders by developing
opportunities to promote growth of subordinates and/or peers.
Exemplary:# Mentors team members to develop their maximum potential
within their scope of practice. 10.# Implements prescribed medical
and psychosocial interventions appropriately to the patient#s plan
of care and applys advanced nursing skills as required, in a
variety of settings. Exemplary:# Participates in the development of
standards, procedures, and/or policies. 11.# Provides and/or
coordinates patient/family education, based on assessed needs, by
creatively adjusting education to the patient#s needs thus ensuring
adequate needs are met. Exemplary:# Analyzes trends in the
patient#s needs, researches literature, contributes to the
development of patient education resources. 12.# Consistently
prepares and administers medications and IV fluids accurately,
according to departmental policies while collaborating with the
healthcare team to provide appropriate medication for patient#s
condition(s).## Exemplary:# Identifies potential problems in
medication administration and makes recommendations for quality
improvement measures. 13.# Consistently anticipates emergency
situations and takes action to prevent or minimize a negative
outcome by demonstrating the ability to organize and delegate
appropriately in emergency situations. Exemplary:# Takes
appropriate action to improve the emergency response system. 14.#
Evaluates the patient#s response to planned interventions and makes
appropriate modifications to the plan of care by collaborating and
communicating with the other members of the healthcare team
regarding changes in the plan of care. Exemplary:# Creatively
designs and implements interventions to minimize the patient#s
length of stay and maintain positive outcomes.#
