• Provide physical therapy services to patients in a SNF / LTC setting
• Develop and implement individualized treatment plans to meet the needs of patients
• Conduct thorough assessments and evaluations to identify patient needs and goals
• Collaborate with other healthcare professionals to ensure comprehensive patient care
• Document patient progress and maintain accurate recordsRequirements:
  • 1 year of recent experience as a Physical Therapist in a SNF / LTC setting
