Description We are seeking a Physical Therapist (PT) for our Outpatient Clinic in Westport, CT. This therapist will collaborate with professionals throughout the facility who provide rehabilitation, pain management, physical medicine, integrative medicine, fitness, nutrition, and behavioral health services to improve surgical and non-surgical outcomes. The Physical Therapist (PT) independently evaluates, determines treatment strategies, delivers and/or delegates services that identify, prevent, remediate, and rehabilitate acute or prolonged physical dysfunction or pain, with emphasis on physical dysfunction. Upon referral, the Physical Therapist (PT) performs thorough examination and evaluation of patient's level of function, develops an individualized patient-centered treatment plan of care, and coordinates patient care with other care providers. Qualifications include being a graduate of a Physical Therapy Program from an accredited American Physical Therapy Association school, with a BS, master's, or doctorate degree, CT active or temporary license, and CPR certification.
