Description

UCFS is looking for candidates who are passionate about making a difference in the lives of others! We are seeking a highly skilled and compassionate Solution Center Registered Nurse to join our call center team. You will be responsible for providing exceptional patient care by assessing and prioritizing patient needs over the phone and will work to ensure patients receive the appropriate level of care.

Essential Responsibilities -

  • Responds promptly to incoming calls and provides nursing triage as calls warrant. Able to provide appropriate triage for pediatric and adult patients.
  • Prioritizes each incoming call level of care to safely meet the needs of the caller (non-urgent, home care, urgent or emergent)
  • Redirects patients who do not need emergency care to their primary care provider. Provides education on home care for minor illnesses. Makes recommendations for further care and when to call if symptoms do not improve.
  • Provides nursing leadership in the patient solution center, instructs, and assists the patient solution specialists and or LPN in proper labeling of calls, emergency calls that require immediate assistance and general nursing triage calls.
  • Collaborates with physicians and other healthcare professionals to ensure patients receive the best possible care
  • Oversees the Chronic Care Management program in basket and directs calls and messages to the appropriate care teams.

    Requirements
    • Graduate of an accredited School of Nursing, BSN preferred
    • One to three years of experience, preferred Ambulatory Care experience
    • Active Connecticut Registered Nurse license
    • Strong critical thinking and problem-solving skills
    • Excellent communication & interpersonal skills to place clients/patients at ease to screen their needs & provide correct service(s)
    • Demonstrated skills working with clients including empathy, exhibiting understanding, patience, and compassion and offering education & reassurance without judgment.
    • Ability to work independently in a fast-paced environment
    • Proficient in electronic medical records systems and has working knowledge of Microsoft Office Suite
