comprehensive and continuous quality improvement information and

benchmarks. Advanced Practitioner Role Scope
- Nurse Practitioners (APRN) / Physician Assistants provide
advanced heart failure consultations within the Skilled Nursing
Facility setting throughout the East Region as well as scheduled
time in the advanced heart failure outpatient clinic located in
Norwich, CT
- Assess newly admitted heart failure patients/post MI to the SNF
setting
- Collaborate with other providers on Cardiac Care with the goal
of reducing hospital readmissions
- Assist in helping to coordinate discharge Advanced

Qualifications Advanced Practitioner Requirements
- Graduate of an accredited Physician Assistant or Nurse
Practitioner program
- Licensed, or eligible to be licensed, as a Physician Assistant,
or Advanced Practice Registered Nurse (APRN) in the state of
Connecticut
- Certification or eligible to be certified as a Physician
Assistant or National Board Certification as an Advanced Practice
Registered Nurse
- Cardiology experience required
- Geriatric board certification preferred We take great care of
