serve.Key ResponsibilitiesAs a Behavioral Health Clinical Team
Manager, you will:
- Plan, direct, and oversee the delivery of home health services
for behavioral health patients.
- Coordinate patient care, including assignments, referrals, and
communication with providers and families.
- Ensure patient care plans are continually assessed, developed,
implemented, and updated.
- Maintain compliance with regulatory guidelines, ensuring timely
initiation of care.
- Process and coordinate physician orders, staff assignments, and
care team communication.
- Review and approve patient care plans and evaluate clinical
appropriateness of changes.
- Integrate input from all involved physicians into a cohesive,
patient-centered plan of care.Qualifications
- Education: Associate's Degree in Nursing or related field
(required).
- Experience:
- 2+ years as an RN in Home Health or Behavioral Health.
- 1+ year of supervisory experience.
- Licensure: Current Registered Nurse (RN) license in [Insert
State].
- Technical Skills:
- Proficiency in Workday and HomeCare HomeBase.
- Proficient with MS Office Suite (Word, Excel, Outlook).Be the
