team, you will: - Conduct comprehensive health assessments,
including obtaining detailed medical histories, performing physical
examinations, and ordering/interpreting diagnostic tests. - Develop
and implement individualized treatment plans in collaboration with
the supervising physician, focusing on health promotion, disease
prevention, and management of acute and chronic conditions. -
Prescribe medications, order therapies, and manage medical devices
according to state regulations and collaborative practice
agreements. - Provide patient education and counseling regarding
health maintenance, disease management, medication adherence, and
lifestyle modifications. - Evaluate and manage common acute
illnesses (e.g., upper respiratory infections, influenza) and
chronic conditions (e.g., hypertension, diabetes, asthma). -
Perform minor office procedures as trained and within scope of
practice. - Collaborate effectively with physicians, nurses,
medical assistants, and other members of the healthcare team to
ensure coordinated and comprehensive patient care. - Document all
patient encounters accurately and thoroughly in the electronic
health record. - Participate in quality improvement initiatives and
adhere to all organizational policies and procedures. - Serve as a
resource for medical staff and participate in educational
activities as needed. - Provide age-appropriate care to patients
across the lifespan, from infants to geriatrics. - Manage referrals
to specialists and coordinate follow-up care. - Maintain current
knowledge of family medicine best practices and engage in ongoing
professional development. Qualifications: - 1-2 years clinical
experience in a of experience in primary care/internal medicine
setting - Active PA or NP in family medicine specialty
certification. - Active, unrestricted CT medical license or in
process of obtaining - ACLS/BLS/PALS (as required) Certification -
DEA License Schedule: 40 scheduled hours, M-F, 8am-5pm, 4-day work
week encouraged Skill Set: Prefer 1-2 years in a clinical working
experience as a NP/PA within the given specialty/service line
recommended. APRNs, PAs, CRNAs and CNMs are required to be
appointed to the Affiliated Medical Staff of a YNHHS Hospital and
