positively impacting the community! Responsibilities About the Role
During visits that may last up to one hour, our providers assess
and observe a member’s health status, medical history, medication
adherence, social circumstances, and other potential risks. This
approach offers invaluable insights into a member’s overall health
that are often missed in standard office visits. The Matrix
Comprehensive Health Assessment (CHA) aims to enhance care quality
and allows us to address multiple care gaps in a single
appointment. Our providers are responsible for: Conducting
assessments for adults and geriatrics, including medical history,
diagnoses, treatment plans, health education, referrals to
physicians and case management, follow-ups, and thorough
documentation in line with our protocols. Administering and
ordering necessary point-of-care and lab tests. Collaborating with
physicians, case managers, social workers, family members, key
caregivers, and other medical personnel as needed. Working
alongside Primary Care Physicians (PCPs) to provide patient
education and necessary follow-ups. Delivering services at the
member’s residence, whether in their homes or Skilled Nursing
Facilities. Benefits of Working with Us Flexibility – Schedule
visits according to YOUR availability. Assessments are conducted
from Sunday to Saturday, between 8 am and 7 pm. You can choose to
work during the day, evening, or weekends based on your
preferences. Competitive Pay – Your earnings are based on the
number of assessments completed. Increase your income potential by
being available for more visits and traveling to see members
outside your home area. Those averaging 20 to 100 visits monthly
can earn between $1,800 and $11,500 , depending on the
per-assessment rate of $90 to $115 based on your state. Benefits –
Includes mileage reimbursement, a 401(k) plan, employee referral

prioritize your growth! Along with comprehensive onboarding
training, you will be assigned a mentor, receive personalized
feedback, and have free access to continuing education
opportunities. Additional Licensure – If needed, we will assist you
in obtaining additional state licensure and credentialing in
neighboring states or others you wish to visit while conducting
health assessments. Full-time Opportunities – Our PRN providers
have the option to transition to full-time roles supporting their
home location or a specific state or region, along with a full-time
sign-on bonus. Qualifications Skills & Experience That Shine A
Master’s Degree or equivalent experience, along with successful
completion of NP or PA licensure. Current NP or PA licensure in the
state of practice, including prescription authority or the ability
to obtain it. Board certification from NCCPA, AANP, ANCC, or AACCN
in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute
Care, Family, Emergency, or Acute Care specialties. Current BLS,
ACLS, or CPR certification. Preferred one year of experience as a
Nurse Practitioner or Physician Assistant; new graduates are
encouraged to apply! A valid state driver’s license and proof of
adequate automobile insurance for your state of residence. Strong
computer skills and familiarity with health/medical record
software. Excellent verbal and written communication abilities. Our
