compassionate and dedicated Nurse Practitioner, (NP) to join our
team. The coverage area for this position is Cheshire and Sullivan
County, NH and Windham Cty VT. Prior home care, hospice, or
palliative care experience strongly preferred, but not required. As
a hospice nurse NP, you will be an integral member of a
multi-disciplinary health care team that provides palliative care
to clients, affording them the opportunity to receive the medical

Responsibilities: - Making home face to face hospice
recertification visits to clients in designated geographic
territories. - Monitoring clients' conditions; reporting changes to
Clinical or Client Services Manager. - Following up with,
executing, and properly documenting doctors' orders. - Performing
client assessments as necessary. Qualifications for a Nurse
Practitioner (NP): - A current license as a NP in NH and VT. - A
minimum of one year of recent, verifiable clinical
(medical/surgical) experience. - Prior home care, hospice and/or
palliative care experience strongly preferred, but not required. -
Solid computer skills; prior experience with electronic medical
records (EMR) preferred. - Ability to travel to cases as assigned.
- Strong interpersonal skills. Base Pay, depending upon
qualifications and subject to negotiation: $70-$80 per hour As an
accredited, regulated, certified, and licensed home health care
