Job Description Job Description Department: Rehabilitation Cherry Brook Health Care Center, a division of New Horizons, Inc., offers short-term rehabilitation, long –term respite, hospice and dementia care. PURPOSE OF YOUR POSITION: Provide direct treatment to a general rehab caseload under the direction and supervision of an Occupational Therapist. Participates in the clinical team process. Provides teaching and training to caregiver staff. Participates in clinical program development. RESPONSIBILITIES INCLUDE: May perform additional assigned administrative duties such as assigned by Rehab Program Manager. Under a physician’s order, provides direct resident treatments in accordance with an established plan of care. Treatment venue includes individual, supervised, and group. Contributes to the team meeting and decision making process as defined by the program. Consults with and trains patients, residents and caregivers. Monitors efficiencies on an ongoing basis in collaboration with supervisor. Documents activities according to the company’s clinical documentation system on a timely basis. Demonstrates consistent knowledge and application of Medicare and other reimbursement regulations. Communicates effectively with patients, residents, team members, customers, and supervisor. Promotes positive customer relations. Participates in clinical program development and implementation. Participates in the training and orientation of other staff, students and volunteers. Participates in census building and marketing activities. Demonstrates ability for continuous new learning to remain current in the field. May perform additional duties as requested by supervisor. Participates in quality improvement activities. Responsible for service and operational excellence of all assigned activities to ensure the delivery of quality services and/or outcomes required to meet or exceed the expectations of those utilizing or impacted by the department. Complies with facility Privacy Practices policies and procedures and protects residents’ individual health information. Exhibits courtesy, compassion, and respect to patients, residents, families, caregivers, visitors, physicians, administrators, co-workers. Knowledge about laws and regulations relating to job responsibilities, e.g., EEO, OSHA, patient rights, fraud/abuse laws. WORKING CONDITIONS: Work is performed in the Rehabilitation Department and on patient care units. Occasional exposure to blood, body fluids, infectious materials, anti-neoplastic pharmaceuticals and other materials and substances that may be used in patient care or on the patient care unit. Standing and walking for significant amounts of time is necessary. Communication skills needed for directing staff, customer relations, marketing activities. Ability to perform documentation needs, manipulations of equipment and direct patient care. Requires intense attention and concentration. Ability to cope with periodic stress associated with changing census and staffing demands. Demonstrated ability to prioritize and manage time effectively for self and others. Ability to deal with a wide range of personality types, ages, and diverse cultural backgrounds. EDUCATION: Associates Degree in Occupational Therapy Assistant from an accredited program. CEU’s per state licensure requirements. EXPERIENCE: Current licensure by the applicable state agency. Two years of clinical experience preferred. SPECIFIC REQUIREMENTS: Must be able to read, write, speak and understand the English language. Strong oral and written communication skills. Strong organizational and analytical skills. Ability to function effectively in multi-faceted systems and to interact effectively with all levels of staff and customers.
