experience Description: We currently have an opportunity for an
experienced RN Care Manager for a 13-week assignment. These are
40-hour per week positions with variable shifts. Shifts may be 8
hours from 8:30 am to 4:30 pm or four 10-hour shifts from 7:30 am
to 5:00 pm. No call is required, but weekend and holiday rotation
is necessary. The RN Care Manager will coordinate concurrent
utilization management activities. This role serves as the main
link between the patient, health care team, organization, and
payors during the patient's hospital stay. Responsibilities include
conducting assessments of patient needs and identifying resources
for successful transitions to the next care setting. Collaboration
and education of an interdisciplinary team in daily care rounds and
care planning are essential. Excellent communication skills in
written, verbal, and electronic formats are required. The RN Care
Manager will work with community organizations to secure necessary
resources, including medical equipment and transportation.
Participation in managing the length of stay of inpatient hospital
populations is expected. Knowledge of hospital reimbursement
methods and quality measures applicable to care management is
essential. Candidates should have either an Associate or Bachelor
degree in Nursing with 3-5 years of clinical experience in relevant
settings. Certification by a nationally recognized case management
