Encompass Health is seeking a dedicated Case Manager to join our team, where we value comprehensive, patient-centered care in healing both body and mind. As a Case Manager, you will play a critical role in coordinating and ensuring continuous care to patients recovering from surgeries, injuries, or dealing with chronic illnesses. Your efforts will be pivotal in helping patients navigate their healthcare journey, optimize treatment efficacy, and improve their overall quality of life. Assess patient needs and develop individualized care plans. Coordinate with healthcare providers and institutions to facilitate effective treatment and follow-up. Act as a liaison between patients, family members, and healthcare professionals to ensure clear communication and proper management of expectations. Monitor patient progress and adjust care plans as necessary. Educate patients and families on healthcare processes, surgeries, treatments, and management of illnesses. Ensure compliance with healthcare regulations and safety standards. Strong organizational and communication skills Ability to handle stress and make decisions under pressure Proficient in healthcare management systems and Microsoft Office Expertise in patient assessment and advocacy Medical Insurance Dental Insurance Vision Insurance 401k Certified Case Manager (CCM) preferred Licensed Registered Nurse (RN) or licensed in a relevant healthcare profession
