This role is pivotal in connecting facility-based and home-based patient care, ensuring a seamless transition and continuous care post-discharge in various locations across 42 states. As a Case Manager, you will be instrumental in evaluating patient needs, coordinating treatment plans, and monitoring progress to optimize health outcomes. Our ideal candidate excels in a multidisciplinary environment and is committed to upholding the highest standards of patient care. Assess patient needs and develop comprehensive care plans in collaboration with healthcare providers. Facilitate communication between patients, family members, and medical staff to ensure understanding and compliance with treatment plans. Monitor patient progress and make adjustments to care plans as needed. Coordinate with healthcare facilities and other agencies to provide necessary services and ensure no lapse in care. Document patient care activities and outcomes for use in performance evaluations and treatment audits. Strong organizational and communication skills Experience with healthcare management systems Ability to work compassionately with diverse patient populations Proficiency in data analysis and patient care evaluation Knowledge of healthcare regulations and accreditation Medical Insurance Dental Insurance Vision Insurance 401k Licensed Registered Nurse (RN) or Licensed Social Worker (LSW), depending on the specific educational background Case Management Certification (CCM) preferred but not required
