Job Description:
Encompass Health is seeking an experienced and dedicated Case
Manager to join our dynamic team, devoted to providing unrivaled
healthcare services. Our Case Managers play a pivotal role in
managing the continuum of care for patients transitioning from
hospital to home. This role requires comprehensive understanding of
healthcare management, exceptional communication, and patient
advocacy to ensure that each patient achieves the highest level of
care, aligned with Encompass Health’s mission of delivering
high-quality outcomes. Assess initial patient needs and develop
individualized care plans. Coordinate with healthcare professionals
to implement effective patient care strategies. Monitor patient
progress and adjust care plans as needed. Provide patients and
families with guidance and resources for optimal care transitions.
Ensure compliance with healthcare regulations and standards.
Document all patient interactions and interventions in healthcare
systems. Strong organizational skills Excellent communication and
interpersonal skills Ability to work independently and make
decisive decisions Knowledge of healthcare regulations and case
management practices Proficient in electronic health records (EHR)
systems Medical Insurance Dental Insurance Vision Insurance 401k
Certified Case Manager (CCM) preferred Licensed Registered Nurse
(RN) or Licensed Social Worker (LSW) depending on state and
position requirements
