Job Description:
AccentCare is seeking a compassionate and dedicated Admissions
Registered Nurse / RN to join our hospice team for weekend shifts.
This role focuses on providing end-of-life care to patients in
their homes, ensuring they receive the highest quality of care
during critical times. The ideal candidate will have a deep
commitment to patient care, excellent communication skills, and the
ability to work autonomously in a dynamic and emotionally demanding
environment. As part of our team, you will play a vital role in our
mission to enhance the lives and well-being of our patients through
personalized, compassionate service. Evaluate patient needs during
the admission process and create individualized care plans. Provide
direct nursing care in accordance with the prescribed plan of care.
Educate patients and their families on the disease process,
end-of-life care, and health maintenance and promotion. Collaborate
with physicians, multidisciplinary team members, and family to
optimize patient comfort. Maintain accurate and detailed medical
records and reports. Administer medications strictly according to
healthcare provider's orders and compliance with hospice policy.
Offer emotional support to patients and their families during the
hospice journey. Strong clinical skills in hospice or palliative
care. Proficiency in patient assessment and nursing documentation.
Ability to manage symptoms and use standard medical instruments and
equipment. Excellent interpersonal and communication skills.
Demonstrated ability to work independently and as part of a team.
Medical Insurance Dental Insurance Vision Insurance 401k Valid RN
license in the state of employment. Basic Life Support (BLS)
certification. Certified Hospice and Palliative Nurse (CHPN)
certification preferred.
