Job Description:
Encompass Health is seeking a dedicated LPN / LVN to join our
dynamic team of healthcare professionals in a post-acute care
setting. In this role, you will be instrumental in providing
high-quality nursing care to patients in accordance with their
individual care plans. This position requires a compassionate
individual who is adept at balancing patient care with medical
protocol while thriving in a multi-disciplinary environment to
optimize patient outcomes. Monitor, record, and report symptoms or
changes in patients' conditions while maintaining accurate and
detailed patient records. Administer nursing care to ill, injured,
convalescent, or disabled patients. Assist in the formulation and
implementation of patient care plans, including administering
medications and assisting with patient rehabilitation. Collaborate
with registered nurses, specialized therapists, and healthcare
professionals to provide optimum care. Educate patients and their
families on health maintenance and disease prevention. Comply with
safety standards, protocols, and procedures to ensure the effective
and safe delivery of patient care. Proficiency in administering
medications and medical treatments Ability to maintain accurate
patient records and document care efficiently Strong interpersonal
and communication skills, capable of managing time effectively
Experienced in patient health education and rehabilitation
Knowledge of disease prevention and healthcare safety standards
Medical Insurance Dental Insurance Vision Insurance 401k Current
Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN)
license in the state of employment. CPR certification
preferred.
