Job Description:
Encompass Health is seeking a dedicated LPN / LVN to join our
dynamic team, providing high-quality nursing care in both
facility-based and home-based settings across multiple states. As a
key player in our integrated healthcare services, you will focus on
delivering exceptional post-acute care, working closely with a team
of healthcare professionals to ensure optimal patient outcomes.
This role includes monitoring patient health, administering
medications, and supporting a multidisciplinary team in a
patient-focused environment. Monitor patient health status and
record patient vitals. Administer medications, injections, and
prescribed treatments. Collaborate with physicians and healthcare
professionals to implement care plans. Educate patients and their
families on treatment protocols and healthcare management. Ensure
compliance with healthcare regulations and safety standards.
Proficient in patient assessment and care planning. Strong
organizational and communication skills. Ability to administer
medications and perform treatment procedures. Knowledge of basic
computer operations and clinical software. Medical Insurance Dental
Insurance Vision Insurance 401k Current LPN or LVN license in the
state of practice. Basic Life Support (BLS) certification.
