Job Description:
AccentCare is seeking a dedicated Clinical Manager / RN in
Hospice care to lead our nursing team in providing exceptional,
compassionate healthcare services to patients in their homes. This
role is pivotal in managing nursing operations, ensuring compliance
with healthcare regulations, and maintaining the high standards of
care that AccentCare is known for. The successful candidate will
collaborate with multidisciplinary teams to optimize patient care
plans and enhance patient outcomes while aligning with our vision
of personalized healthcare services. Oversee the day-to-day
clinical operations in the hospice program. Supervise and mentor
hospice nursing staff and caregivers. Ensure compliance with all
local, state, and federal regulations. Coordinate training programs
for all clinical staff. Evaluate patient care plans and monitor
outcomes. Facilitate multidisciplinary team meetings to discuss and
optimize patient care. Manage patient and family relations and
support best practices in end-of-life care. Strong leadership and
organizational skills Excellent communication and interpersonal
skills Proficient in healthcare management systems and electronic
health records Adept in conflict resolution and patient care
management Able to handle stressful situations and make decisions
promptly Medical Insurance Dental Insurance Vision Insurance 401k
Valid Registered Nurse (RN) license in practicing state.
Certification in Hospice and Palliative Nursing (CHPN)
preferred.
