Job Summary We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our dynamic agency. In this vital role, you will coordinate patient care across various settings, ensuring seamless case plans and home health care planning. Responsibilities Develop personalized care plans that incorporate discharge planning, and creating case plans for in home health care. Collaborate with multidisciplinary teams—including operations, rehab facilities, therapists, social workers, and other healthcare professionals—to coordinate patient treatment plans. Manage utilization through effective case management practices, ensuring appropriate level of care while adhering to HIPAA regulations. Facilitate intake processes for inpatient and outpatient services, including nursing homes, rehab and skilled care facilities. Ensure compliance with policies, infection control standards—particularly in infectious disease care—and safety protocols across all patient interactions. Experience Proven experience in home health care, hospital or rehabilitation is preferred. understanding of trauma protocols and emergency medicine procedures. Hands-on experience in hospital medicine, medical-surgical units, or emergency room settings demonstrating advanced patient assessment skills. Background in post-acute care coordination including discharge planning for complex cases involving geriatrics or behavioral health populations. Familiarity with managed care principles and utilization management processes to optimize resource use without compromising quality of care. Knowledge of physiology and medical terminology essential for accurate case documentation and diagnostic evaluation. Experience working within hospital or outpatient clinics—particularly in pediatrics (PICU), Geriatrics, or outpatient settings—is highly desirable. Ability to navigate electronic health records efficiently while maintaining strict compliance with HIPAA standards.
