Job Description:
The Bridge Family Center is currently searching for a full time
Transitional Living Coordinator to provide overall case management,
behavior management, and life skills instruction for its Moving On
Project, a transitional living program for DCF involved adolescent
males. The Bridge Family Center offers competitive pay and great

sick time, personal time, and 8 paid holidays. ESSENTIAL DUTIES AND
RESPONSIBILITIES: I. Case Management Establish and maintain regular
contact with DCF, schools or educational program, employer, and
significant others involved with the residents. Meet with assigned
residents individually on a weekly basis. Oversee the maintenance
of assigned cases relative to incident reports, legal documents,
case management notes, resident appointments, and any other
required information. Attend all pertinent meetings/conferences
related to assigned cases. Work closely with the Program Director
in formulating effective, goal-oriented Individual Transitional
Living Plans with assigned residents. Act as a team member to cover
for other case managers. Participate in the instruction of the
Basic Life Skills Course to all residents. Implement incentive and
reward system to motivate residents. Engage guest speakers and plan
workshops for residents. Develop individual goals to ensure
resident learns to live independently. II. Behavior Management
Monitor daily behavior of assigned residents through
interaction/discussion about daily activities. Use appropriate
judgment in connection with appropriate discipline as required,
keeping participants in line with program guidelines. Assist
clients in daily living skills, as needed. Conduct apartment spot
checks. III. Program Management Assist with intakes and the
discharge of participants in the program, which includes data entry
of required information for tracking purposes. Provide crisis
intervention counseling as necessary. Transportation of clients, as
required. Prepare monthly report of caseload for DCF as per
assigned deadline, which includes summary of each individual
relative to life skills, money management, interpersonal skills,
etc. Administration of medication; maintenance of appropriate
documentation. General maintenance of daily/shift documentation.
Ensure program compliance at all times. Participate in weekly team
meetings to review each participant and determines appropriate
courses of action. Provide adequate supervision at all times
(minimum ratio 1:6), to ensure resident support and safety.
Demonstrate positive rapport with residents, including establishing
professional boundaries. Exercise effective group skills and
techniques. Other duties as assigned. Qualifications: Bachelor’s
Degree in Human Services or related field highly preferred.
Experience working with adolescents as well as case management
experience required. Must have ability to set professional
boundaries and limits, and possess excellent communication and
interpersonal skills. Must have strong documentation and both
verbal and written communication skills. Must also have a valid
Connecticut driver’s license with clean driving record, and possess
the following certifications, or be eligible to obtain these after
hire: DCF Medication Administration, TCI, and First Aid/CPR.
