If you are looking for an opportunity to support children in a psychiatric residential treatment facility and their families learn the skills needed to live together successfully, the Transitional Care Coordinator position is for you. JOB SUMMARY The Transitional Care Coordinator (TCC) is responsible for ensuring the successful transition of clients from the Psychiatric Residential Treatment Facility (PRTF) back to the community. In partnership with the PRTF staff, the TCC provides a range of case management, in-home support, mental health, and after care services to children and families in the PRTF. This position requires some flexibility to work some early evenings and weekend hours. RESPONSIBILITIES: Work with a case load of 6-8 clients/families. Provide intensive in-home, case management, support services, advocacy, trauma informed psycho-education and parenting skills training/teaching, coaching, crisis support, and advocacy to families Work as a member of an integrated system of care, in collaboration with parents, PRTF staff, DCF and other community providers to support family. Provide on-call therapeutic support to families, in conjunction with the clinician, working with clients in crisis during home visits; Maintain documentation according to licensing, accreditation, and Medicaid regulations as appropriate. Participate in the implementation of treatment plans, suggesting modification as appropriate during the course of service, to ensure treatment gains made in the PRTF program transfer to the community setting/home. Assist families with identifying basic needs; providing or accessing transportation; accessing school, job preparation, employment and housing information; locating free family recreation activities in preparation for the clients return to the community Compile and update information on resources in the local area for the team. Attend and participate in weekly rounds and child specific team meetings Develops and maintains collaborative relationships with other community agencies and organizations in consultation with clinician and supervisor, in order to provide comprehensive and effective treatment services for clients (i.e. schools, courts, hospitals, etc.) Other duties or responsibilities as determined by management. Experience and Skills: BA in the human services field or BSW required; 3 years experience in a comparable position in a human services agency preferred. Required to maintain valid drivers license and ability to drive. Excellent oral and written skills and ability to communicate comfortably with children, parents, and school personnel Organized and able to work independently with minimal supervision. Flexible and able to multi-task. Cultural competency to serve targeted populations. Bilingual skills preferred, but not required Experience working with inner-city children. Commitment to Results/Performance Based Accountability. Proficient with word processing and spreadsheet applications. Possess solid general computer skills, including ability to work in a Windows XP environment. Familiarity with specific applications used by the Village a plus. Physically able to perform the essential functions of the position, including performing physical holds if needed for safety, with or without reasonable accommodations; able to function safely in a therapeutic situations including provision of services in clients homes or host settings; able to travel to various sites throughout CT.
