Bi Lingual Housing Support Services Specialist/Care Manager
Connecticut Community Care is looking for a Bi-Lingual Care
Manager/ Supportive Services Coordinator to works in cooperation
with the on-site management staff of Huntington Towers. You
position will work to improve the quality of life of the residents
by providing social, educational, and recreational programs and
services. Assists individuals in maintaining an interactive process
of informed decision-making about Long-Term Services and Supports.
Serves a key role in coordinating the efforts of formal and
informal caregivers on behalf of clients. Supports up to 50 clients
as the primary Care Manager for those on DSS public programs who
reside in Huntington Towers.
In conjunction with management at Huntington Towers, conducts
resident assessment at move-in and at least on a yearly basis to
identify community needs and interests.
Through referral from management staff, conducts one-on-one
assessments, determines intervention measure, provides appropriate
referrals to social service and/or human resources agencies, and
conducts follow-up assessments.
Educates individuals on the components of the program, service
options, and DSS guidelines, including eligibility, costs, how each
may work with the persons formal and informal supports and
resources, and the pros and cons/costs and benefits of each
Monitors and reviews continued cost effectiveness, quality and
appropriateness of care plan/service delivery, service order entry
and renewals, and the contractual obligations. Works with the
individual to make revisions where necessary, at established
intervals and as otherwise indicated, in conjunction with the

disputes between residents, including problem solving, conflict
mediation and resolution through written agreements. Works in
collaboration with Huntington Towers management to resolve
disputes. Attends meeting and provides support/follow-up with
Works with residents to: find new physicians; resolve utility
issues; State Entitlement Assistance (i.e. T-19, SNAP, Housing);
makes referrals to CHCPE and other programs as appropriate; assists
with citizenship questions and transportation requests.
Other duties as necessary
Requires a bachelors degree in administration, social work,
nursing, public health, psychology, counseling or gerontology or
related field required.
Must have a minimum of two years experience in health care or human
services.Previous experience as a Resident Services Coordinator or
in a similar role.Knowledge and understanding of psychological,
human development, social, health, and economic factors influencing
the attitudes and behavior of individuals and families, especially
as they relate to the geriatric and disabled populations; knowledge
and skill in interviewing and assessment (social and health)
techniques; understanding of chronic illness and its effect on the
individual and family.
Bilingual in English/Spanish required.
