Job Description Job Description The Care Coordinator provides
family-centered planning facilitation, develops plans of care
driven by the strengths and needs of the child, youth, and
family/primary caregiver, obtains targeted support services, and
monitors the provision and outcomes of plan so that the child or
youth and family can be successful at home, at school, and in the
community. To be effective, the coordinators will work with
children/youth in their homes and communities, incorporate the
entire family and their natural supports in planning process, focus
on the child’s and family’s strengths, and demonstrate cultural and
linguistic competence. Essential Job Functions · Maintains a
caseload of 12 in weekly face-to face wraparound planning and team
meetings. · Engage family in New Hampshire Wraparound process to
identify their unmet needs, functional strengths and individual
supports that are vital to improving the daily functioning. ·
Develops and supports a family based Plan of Care in accordance
with New Hampshire Wraparound Manual and Curriculum. · Coordinates
and monitors service between agencies to meet the identified
individual needs. · Assists youth and family in identifying natural
supports and community based opportunities and activities (sports,
arts, clubs, and volunteerism) that nurture strengths, skills, and
resilience, and provide support for youth and family to be fully
engaged in their community. · Establishes a trusting relationship
with the child/youth client and family. · Schedules, coordinates
and facilitates family team wraparound meetings. · Adheres to the
New Hampshire Wraparound Model to maintain the highest fidelity in
service to those values and principles. · Enters all appropriate
case specific information into the data system. · Adheres to
ethical, required reporting, and confidentiality standards as
required by state laws and New Hampshire Wraparound standards.
Qualifications/Education/Training · A minimum of a Bachelor’s
degree from a recognized college or university in related field. ·
A minimum of two years’ experience, preferably in health care,
social services, behavioral health care, social work, human
services, or education, with care management experience. · A high
school diploma or equivalency; and a) was a participant in, or is a
direct caregiver, or was a direct caregiver of an individual who
received services from the public child and family-serving system;
and b) meets the training and certification requirements for the
care coordinator. Other Duties and Responsibilities · Adheres to
County and program policies and procedures. · Extensive regional
travel required. Occasional statewide travel and attendance at
out-of-state Substance Abuse and Mental Health Services
Administration conference required. · Must maintain valid drivers
license, current NH vehicle registration and inspection as well as
insurance with a minimum of $50,000/$100,000 for bodily injury &
